Scheduling An Appointment
To help provide timely service to our patients, you can book well/annual visits, shot visits only and sick visits online or by calling our office. All other visits you should call the office and press option 2 for the front desk team.
We have no grace period for being late for your appointment, we will try to work you back into the schedule, or perhaps offer an appointment to you with another provider.
Out of courtesy to our other patients, we may be forced to ask you to reschedule your appointment. Please understand that showing up late for your appt may result in a no show fee if we cannot accommodate you in the schedule that day.

THINGS TO KNOW FOR YOUR UPCOMING VISIT:
We have partnered with CHADIS to allow us to have all of those important developmental and mental health screenings completed PRIOR to your visit to minimize your time in the office and having to touch pens/paper! Completing these advanced tools PRIOR to your visit allows us to have them scored and ready to discuss with you at the visit. You can log in to your patient portal up to 2 weeks in advance of your appointment to complete all screenings. CHADIS forms are done at every well visit and some other visit types as well. A step by step guide on how to access these forms:
Appointment Required
All visits require an appointment except in the case of an emergency. We do offer walk in telehealth appts during specific posted hours. Unfortunately, we are unable to see your child if you walk in without an appointment. We will ask you to return later in the day when the schedule allows.
Sick Appointments
Learn MoreYou can book online sick appts or call the office to schedule an in person. We also have walk in sick appts by telehealth.
If you have a medical emergency during office hours, we will do our best to see your child immediately or refer you to the appropriate facility.
Well Appointments
Learn MoreDid you know regular well child visits are one of the most important things you can do to keep your child healthy? Well child visits also provide you with the opportunity to discuss any questions or concerns you may have about your child’s health.
Your child should have a well child visit at the intervals listed below. We recommend that you call our office 3 months in advance to schedule an appointment with your child's primary care provider on the date that best fits your needs. Or, you can book well visits on our website.
